Additional Membership Details

Every membership includes your choice of one of the following wellness injections once a month:

  • Lipo-C injection (Methionine/Inositol/Choline/L-Carnitine/Cyanocobalamin; 25/25/50/100/1mg/ml)
  • Nitro Aminos injection
  • B-12 injection
  • MIC (Methionine/Inositol/Choline/Cyanocobalamin; 25/100/50/1mg/ml)

How Our Membership Works

All membership fees and payments apply to your services and products:

  1. Membership renews automatically every month.
    1. 90-day minimum commitment is required
    2. After 90 days, the client may cancel or change tiers with 30-day written notice
  2. Membership fees are non-refundable, including unused injections or discounts.
  3. “Wellness injection” refers to one provider-administered injectable treatment per month, using a formulation appropriate for wellness or aesthetic support, as determined by the provider following medical evaluation.
    1. Alternative formulations or upgraded dosing may incur additional charges.
  4. Monthly injections expire six months after issuance.
    1. IVs typically expire six months after issuance
    2. Discounts cannot be carried forward after cancellation
  5. Membership benefits, services, and discounts are non-transferable.
  6. Membership discounts cannot be combined with other promotions unless explicitly stated.
    1. Discounts apply to regular pricing only
  7. 24-hour notice is required to cancel or reschedule appointments.
    1. No-shows or late cancellations may result in forfeiture of that month’s injection or a fee of $25
  8. Medical eligibility means all treatments require provider approval. If a client becomes medically ineligible to receive injections, payments remain non-refundable but may be applied to alternate services at provider discretion.
  9. Membership pricing and benefits may change with 30-day written notice.
  10. Client agrees to sign all required medical consent forms prior to receiving treatment.
